PARTICIPANT ELIGIBILITY CRITERIA:

• Must be at least 60 years of age and ineligible for OCTA ACCESS..
• Reside in Anaheim, Anaheim Hills, Brea, Fullerton, La Habra, La Palma, Placentia, Yorba Linda , and Villa Park .
• Please call Office on Aging at (800) 510-2020 to be referred. The Office on Aging Information line can also provide referrals for additional community resources. Senior Connections direct line is (714) 446-7151.

SERVICE CRITERIA:

• Service is available for transportation to medical appointments, therapies, hospitals, senior centers (when participating in health improvement/education programs), dental offices and Walgreen drive-thru pharmacies. Prescriptions must be phoned in prior to leaving physicians office. Please note that we are not able to respond to emergency calls.
• Services must originate in Orange County and trips are limited to 15 miles from the participant's residence.

TRANSPORTATION HOURS:

• 7:00 a.m.-6:00 p.m. Monday thru Friday
• Transportation is not available on major holidays.

FEE PER TRIP:

• $2.00 per trip (based on ability to pay)
