PARTICIPANT ELIGIBILITY CRITERIA:

• Must be at least 60 years of age and ineligible for OCTA ACCESS.
• Reside in Costa Mesa, Cypress, Fountain Valley, Garden Grove, Huntington Beach, La Palma, Los Alamitos, Midway City, Santa Ana, Seal Beach, Stanton, Tustin, Westminster.
• Please call Office on Aging at (800) 510-2020 to be referred. The Office on Aging Information line can also provide referrals for additional community resources. St. Anselm direct line is (877) 777-0988.

SERVICE CRITERIA:

• Free transportation to medical & dental appointments, exercise programs, outpatient services, lab testing, mental health appointments, rehabilitation & therapy, health education and prescription pick ups.
• Services are limited to the designated service area and a 15 mile radius from participants home.

TRANSPORTATION HOURS:

• 8:00 a.m.- 5:00 p.m. Monday thru Friday.
• Transportation is not available on major holidays.

FEE PER TRIP:

• $2.00 per trip (based on ability to pay)
