PARTICIPANT ELIGIBILITY CRITERIA:

• Must be at least 60 years of age and ineligible for OCTA ACCESS.
• Reside in Costa Mesa, Fountian Valley, Garden Grove, Huntington Beach, Santa Ana or Westminster.
• Service area covers cities listed above as well as a 15 mile radius from the participant's residence.
• Please call Office on Aging at (800) 510-2020 to be referred. The Office on Aging Information line can also provide referrals for additional community resources. VNOC direct line is (714) 558-3097.

SERVICE CRITERIA:

• Transportation to medical & dental appointments, exercise programs, outpatient services, lab testing, mental health appointments, rehabilitation & therapy, health education and prescription pick ups.
• Services are limited to the designated service area.

TRANSPORTATION HOURS:

• 8:00 a.m.- 5:00 p.m. Monday thru Friday.
• Transportation is not available on major holidays.

FEE PER TRIP:

• $2.00 per trip (based on ability to pay)
