Healthy Living with Chronic Conditions Program

The Healthy Living (or Chronic Disease Self-Management Program) is a FREE evidence-based program developed by Stanford University and has been shown to improve health and well-being of adults with different chronic conditions. The Healthy Living program is offered free to Orange County residents over the age of 18 years old.

About The Healthier Living Program Workshop:

  • Meets 2 ½ hours per week for six weeks
  • Small group (10-15 participants)
  • Peer-led (non-health care professional) self-management workshop
  • Promotes empowerment by teaching behavior management
  • Not disease-specific
  • Great for people with a single or multiple chronic health conditions
  • Great for caregivers
  • Addresses topics such as
    • Problem-solving
    • Relaxation techniques
    • Nutritious eating
    • Improving communication with physicians, family, and friends
    • Medication “how to’s”
    • Weekly goal setting

Interested In Becoming a Host Site for the Healthy Living with Chronic Conditions Program:

The Office on Aging is accepting inquiries from local Orange County community based organizations that are interested in hosting the Healthy Living 6-week program. Currently the Office on Aging is working with local hospitals, community and senior centers, churches, and libraries to facilitate the Healthier Living program. It is a wonderful opportunity for community organizations that provide direct services to adults or caregivers. If you would like to be involved in hosting these workshops and are able to:

  • <Provide private meeting space for at least 20 people
  • Be able to provide classes on the same day for six consecutive weeks for 2.5 hours
  • Can help support and promote the Healthier Living program by distributing flyers, displaying flyers in the organization's lobby/reception area, and/or list the program on the organization's website/calendar of events

Help Yourself and Others with Ongoing Health Conditions By Becoming a Leader!
 

Do you have a desire to help others? Do you have your own ongoing health conditions? How about helping yourself and others by becoming a leader for  the Healthy Living With Chronic Disease Self-Management Program. Contact the Orange County Office on Aging to find out when the next leaders training is.

 

Interested Leader applicants must: 

  • Attend a four-day training (9:00 am to 4:00 pm)
  • Be comfortable in front of a group
  • Able to facilitate future Tomando de su Salud workshops
  • Speak Spanish fluently

Volunteers who are interested in becoming a trained leader are to complete a Registration Form Application and return it to the Office on Aging at: officeonaging@ocgov.com OR
